Introduction

 
The U.S. Government Accountability Office (GAO), an agency of the Congress, is mandated by the ENHANCE 911 Act of 2004 (Public Law 108-494) to review wireless E911 funds collected by states and local jurisdictions. As part of our review, we are conducting a web-based survey that will allow us to obtain information on the states' collection and use of wireless E911 funds. This survey will collect information from E911 contacts in all 50 states and the District of Columbia. We plan to issue our report to the Congress early next year.

Please complete this questionnaire within 2 weeks of receipt. It will take about 15-30 minutes to complete. We understand that there are great demands on your time.
