How to fill out this questionnaire:
  • Click buttons with your mouse.
  • Enter your narrative answers into text boxes by clicking inside the boxes and typing; they will accept more text than fits within the visible box.
  • When you choose an answer that instructs you to skip to another question, you can do that by clicking the "Click here to go to Question #" link.
How to Move Around:
  • If all the questions on a page don't fit on your screen, slider bars will be visible on the bottom and side of your screen. Reposition the page as necessary.
  • Use the "Next" and "Previous" buttons at the bottom of each page to move forwards and backwards through the questionnaire. The arrow icons at the bottom of your screen can be used to jump to the beginning or end of the questionnaire.
  • Do NOT use your web browser's arrows or the "Enter" key on your keyboard to move from page to page.
How to Print Your Answers:
  • The last page has a link to print your answers to the questionnaire.
  • You can also print each individual page before or after you have filled it in with your answers with the "Print" button at the bottom of that page.
How to Save Your Answers and Come Back Later:
  • To save and temporarily exit the questionnaire at any time, press the "Exit" button at the bottom of a page.
  • To re-open your questionnaire and enter more responses or change previous responses, come back to the web address we gave you and re-enter your username and password. The questionnaire will restart at the last question you answered.
  • Always use the "Exit" button to close the questionnaire. If you do not, you will lose information you entered on your current page.
How to Tell Us You're Done:

The last question asks you to tell us when you have completed the questionnaire and that GAO has permission to use your answers. We won’t use your answers until you have done this.
