To save and temporarily exit the questionnaire at
any time, press the "Exit" button at the bottom of a page.
Always use the "Exit" button to exit.
If you do not, you will lose information you entered on your current page.
To re-open your questionnaire and enter more
responses or change previous responses, return to the Web
address we gave you and re-enter your username and password.
The questionnaire will restart at the last question you answered.
You can move forwards and backwards in the questionnaire at any time to revisit parts of the questionnaire and change answers by using the menu window on the left or the buttons at the bottom of each page.
