To fill out this questionnaire: Answer closed-ended questions by clicking buttons
or boxes with your mouse. To answer short answer or narrative questions, click anywhere inside the
box and type your response. For narrative questions, the box will expand to accommodate long
answers.
To change answers: To change an answer marked with a "button" (circle), click on
another answer. To uncheck a box that you checked, just click on that box again.
To move from section to section: Click on the "Next section>" or "<Previous section"
buttons at the bottom of each screen. You may also use the "Sections" menu bar on the left of the
screen or the double arrows at the very bottom of each screen.
To save your answers and come back later: You may exit the questionnaire at any time by
clicking on the "Exit" button at the bottom of each screen. When you log on again, you will be sent
directly to the last question you answered. You may also go back in the questionnaire and edit
previous responses. Always use the "Exit" button to close the survey. If you do not, you will lose
the information you entered in that section.
To print your responses: You may print your responses for each section by clicking on the
"Print" button at the end of each section. The last screen also provides a link if you want to print
the responses from your entire questionnaire.
Pop-ups: Various terms used in this questionnaire are defined in pop-up boxes. To open a
pop-up box, click on the blue or violet underlined text and the box will open up. You may print the content
of the box on your printer if you'd like. The definitions contained in the pop-up boxes are also found on
the last screen of this questionnaire under the heading "Definitions."
To complete the questionnaire and tell us you are done: The last question (question #46) asks you
to confirm that you have completed the questionnaire and that GAO has permission to use your answers. We
will not use your answers until you have completed this question.
