Background

  The Chair, U.S. Senate Special Committee on Aging, and the Ranking Member, Senate Committee on Finance, have asked the Government Accountability Office (GAO) to study and report on the process for identifying and citing nursing home deficiencies. GAO is requesting that surveyors in all 50 states and the District of Columbia complete this brief questionnaire about the survey process in their states. Your responses will provide valuable information about the factors that make up deficiency determinations.

GAO will generally report the results of this questionnaire in the aggregate. While it may incorporate individual responses in the report, it will do so in a manner designed to ensure that individual respondents cannot be identified. Further, it will only release individually identifiable versions of responses if required by law, if exceptional circumstances indicate review by another agency is appropriate, or if requested by a member of Congress.

On the basis of initial testing, GAO expects that it will take approximately 15 minutes to complete this questionnaire. Please submit the completed questionnaire within 2 weeks.
