Introduction

The U.S. Congress has asked the Government Accountability Office (GAO) to review the U.S. government's Feed the Future (FtF) Initiative. As part of this review, GAO is sending this questionnaire to all key government representatives of various U.S. agencies who are providing support to FtF-related activities and projects in the 19 FtF countries. Please answer the questions based on your individual experience and point of view based on your tenure in _____, not based on the entire FtF program at your agency or mission. Answering these questions will help us inform Congress about this important issue.

Responding to these questions should take about an hour and can be completed over multiple sittings. Your responses can be saved and accessed at a later date. To learn more about completing the questionnaire, printing your responses, and whom to contact if you have questions, _____. If you are unsure of how to respond to a question, please contact us for assistance. Important definitions used throughout the survey can also be found _____.

We will not release outside of GAO any individually identifiable data obtained from responding to this questionnaire, unless required by law or requested by a member of Congress. We will generally report the summary results of this survey. If we incorporate examples of individual survey responses into the report, we will do so in a manner that prevents the identification of the respondents.

Thank you for your time and assistance.

NOTE: Acronyms stated in some questions' results are spelled out in the full survey question, which can be viewed by clicking on the View Question link.
