Surveys of State Regulators on the Death Care Industry

	When the state officials that we identified as our points of contact logged onto our survey website, they were presented with a menu screen that indicated to them which of the five surveys they were being asked to complete. When they clicked on each of these links, the survey for that segment of the death care industry opened and they were then able to complete it.
